Structured Family Caregiving Waiver

Caring for a loved one with dementia at home.

When Alzheimer’s or a related dementia means a loved one needs nursing-home-level care, Missouri’s Structured Family Caregiving Waiver can support a live-in family caregiver instead.

What Is the Structured Family Caregiving Waiver?

The Structured Family Caregiving Waiver (SFCW) is a Missouri Medicaid program for adults age 21 and older living with Alzheimer’s disease or a related dementia who would otherwise qualify for care in a nursing facility. It allows them to remain at home with a live-in primary caregiver who provides daily care and supervision.

Who Can Be the Caregiver

  • An eligible family member, non-family member, or legal guardian
  • Must live full-time in the same home as the participant
  • A backup (substitute) caregiver must be identified
  • The caregiver may hold another job if it doesn’t interfere with care

Why Families Choose This Path

For someone living with dementia, familiar faces and surroundings matter. Staying home with a trusted family caregiver can bring comfort, reduce confusion, and keep your loved one close to the people and places they know.

Who May Qualify

The participant generally must be 21 or older, have Alzheimer’s disease or a related dementia, meet nursing facility level of care, and have qualifying MO HealthNet coverage. Program rules can change, so call us to review your situation.

FAQ

Questions Families Ask Us

Can I get paid to care for a family member with dementia in Missouri?

The Structured Family Caregiving Waiver supports an eligible live-in caregiver for a qualifying person with Alzheimer’s or a related dementia. Both the participant and caregiver must meet program requirements.

Does the caregiver have to live with my loved one?

Yes. SFCW uses a live-in model. The participant lives full-time with the primary caregiver, in either person’s home.

How is SFCW different from Consumer Directed Services?

CDS lets an eligible participant direct their own personal care and choose an attendant. SFCW is specifically for people with Alzheimer’s or a related dementia who live with their primary caregiver. A participant in SFCW generally can’t be enrolled in another waiver program at the same time.
